Carmelita C Divinagracia, a nurse instructor at the University of Santo Tomas College of Nursing in Manila, Philippines is said to have developed the theory that "nursing is not an art but it is a science." This means that nursing does not have its own unique qualities or particular elements. Rather it can be decomposed into five main components. A teacher's role in this theory consists of utilizing each one of these five components appropriately to ensure optimum patient care. These are observation, assessment, formulation and management, implementation and evaluation. The observation component involves getting general details about the present condition of patients just by looking at them without engaging the patient further in any conversation. The assessment component involves the patient's physiological status, general medical history, medications including their dosage, special diets and treatments considered beneficial to the patient. The formulation component is the basis of the nursing diagnosis that is assigned by a practicing registered nurse to each patient. It consists of clinical observations, physical examination and laboratory tests. The use of regular diagnostic criteria such as those in Diagnostic and Statistical Manual for Mental Disorders (DSM-IV) is recommended. The management component is the most difficult task in nursing. It has to be carried out by a nurse who acts as a caregiver, an extension of the patient's family.
